If you have bought the Premiership DVD Collection in a shop you must register it here so that we can record your name and the address you would like the remaining 34 matches to be sent to.

When you bought the Collection you received a Collectors album and a paper wallet containing the first DVD and a Registration Certificate.

Please fill in this form quoting the Unique Registration Number on the Registration Certificate and we will send the remaining 34 matches to the address you specify (only UK addresses are accepted).

Alternately call 0870 11 88 030 (Monday – Friday 9.00am – 5.00pm) and register by telephone.

This service is only available in the UK and mailing addresses outside the UK can not register.
